Visa Application Checklist
The employee will need to provide the following information.

2 recent passport size photographs (45mm x 35mm) of the employee and any other person included in the application. The photos should be of the head and shoulders only, and should show the person facing the camera and against a plain background. Print the name of the person on the back of each photograph. 2 additional photos of each person are required if health examinations have not been completed. |

| If anyone included in the application is or has been married, certified copies of the marriage certificate(s). | |
| If anyone included in the application has been divorced or widowed, a certified copy of the divorce decree absolute or the death certificate of the deceased partner (as appropriate). | |
| If anyone included in the application has changed his or her name (for example, by marriage or deed poll), a certified copy of evidence of the name change. | |
| Certified copies of birth certificates or the family book, showing names of both parents, for all the children included in the application. | |
| If any child included in the application is adopted, certified copies of the adoption papers. | |
| Certified copies of documents to verify custody and access arrangements for children under 18 years unless both parents of the child are included in the application. | |
| Health and Character requirements. | Completed health examinations for all persons included in the application (Only if the employee is inside Australia when the application is lodged). See: Form 1071i Health requirement for permanent entry to Australia (57KB PDF file) |
| A police good conduct/character certificate for each person aged 16 years or over included in the application, for each country, including Australia, lived in for more than 12 months over the past 10 years. See: Character and Penal Clearance Requirements |

| If the employee has served in the armed forces of any country, certified copies of military service record or discharge papers. | |
| Qualifications and experience | Certified copies of all qualifications and work references. If applicable, evidence that the qualifications are recognised in Australia. If this is required, it must be provided with the application at the time it is lodged. |

| Evidence from a relevant Australian registration or licensing authority that the employee holds necessary registration or licensing requirements (if applicable). | |
| A formal letter of appointment or contract covering the position for which the employee has been nominated. | |
