If you completed the online SGP application form your Login ID and password for the online OSCAR data entry function remains the same.
If you do not have a Login ID and password, please complete the following form and fax it to us.

Saving online forms (OSCAR and application)
Online forms automatically time out after 20 minutes if you have not:
- saved your work
- clicked on any of the buttons
or - selected a menu item.
Remember to save your work regularly.
